If you're playing Animal Hospital on Roblox, sooner or later you'll admit something that wasn't a dog. The game is basically a vet clinic night shift where some of the patients are skinwalkers, and one wrong admit can wreck your whole run.
This guide lists every anomaly sign in the game and how to check for them, plus what to do about the stuff crawling around the hospital while you work.
Quick note before we start: the game updates constantly, so a tell or price here might shift from patch to patch. The core checking system hasn't changed though.

The 3 checks (do all of them, every time)
Every patient at the intake desk gets three checks. Not one. Not two.
- Window - look at them. Physical tells show up here.
- Photo - take their picture. Some fakes look fine in person but not on film.
- CCTV - check how they acted on the way in.
Each check catches things the other two miss, which is the whole trick of the game. One failed check = reject. Hit the shutters and move on. There are no "maybe" cases.
Early shifts, the fakes are lazy and obvious. Around shift 4 they get good. That's when people start losing runs because they got comfortable and skipped the photo.
All anomaly signs
Window tells
- Three eyes
- Hollow / empty eye sockets
- Sharp teeth on an animal that shouldn't have any
- Weirdly realistic (human) teeth
- A grin that's too wide
- Twitching
Photo tells
- Wrong photo - picture doesn't match what's in front of you
- Static photo - comes out grainy and messed up
- Cursed photo - bloodshot eyes and a grin that isn't there in person
CCTV tells
- Black box censoring the eyes
- Stretched or distorted limbs
- Staring straight into the camera
- Body completely blacked out
One thing about the cameras: don't sit on the CCTV feed. Check, confirm, get off. Staring at it too long attracts attention you don't want.
The window blocker
Sometimes a creature parks itself at your window and blocks the blinds so you can't check anyone. Here's what NOT to do first:
- Asking it to leave costs you sanity. A lot of it, up to around 20 points.
- Tasering it costs you a chunk of health.
The actual answer: give it coffee. A few sips and it leaves on its own. That's it. Keep one spare coffee on you at all times just for this, even with full sanity.
Treating actual patients
Once someone real is admitted, they get sent for treatment:
- Rooms 1-5 - normal care. Run the DNA analysis, give the items it asks for.
- Room 6 - X-Ray. Sequence-matching mini-game.
- Room 7 - Heart monitor. Click the icons.
- Room 8 - Surgery. Timed, and you can't leave the room until it's done. Even when something shows up to mess with you. Especially then.
Three dead patients in one shift ends your run. An ignored patient kills you just as dead as a monster does.
Everything hostile inside the hospital
Rejecting fakes at the desk doesn't keep the building clean. Responses for everything else:

The fire extinguisher ends up doing way more work than you'd expect - fires, ghosts, slime. Buy it early.
Mid-shift events
All of these are on timers:
- Fainted patient - ~60 seconds to get them in a bed
- Patient on fire - extinguish, then Ointment
- Death ritual - ~60 seconds to put out the candles
- Critical patient - ~2 minutes to the recovery room
- Room fire - spreads if you ignore it
- Ambulance (shift 4+) - a pile of patients at once. Stop intake, deal with it
When an event pops, stop taking new patients. A minute of closed intake costs nothing compared to a dead patient or a burning ward.
Sanity and what to buy first
Sanity is your real HP. Jumpscares, hostile encounters, and general stress drain it. Zero = shift over, instantly.
Coffee and chocolate restore it. Tasering an anomaly successfully also gives some back, which is a nice bonus.
You get cash every shift. Spend it in roughly this order:
- Taser - stuns anomalies, refunds sanity on a hit
- Coffee + chocolate - never start a shift without at least one of each
- Fire extinguisher - solves three different problems
- Second intake window - doubles registration speed, massive later on
- Maple Syrup / Ointment - cheap, and you'll want them when you want them
- Firearm - it works, but no sanity refund. Luxury item
Solo vs. with friends
Solo: don't bounce between the desk and the treatment rooms, you'll do both badly. Register a batch, then treat everyone in one sweep. Keep checks around five seconds per visitor, but never skip a step to save time.
Co-op: split the jobs. One person owns the desk and cameras, one owns treatment, and a third floats for events and anything that got inside. Two players? The floater job goes to whoever's less busy.

FAQ
What happens if I admit an anomaly? It gets inside and you deal with it mid-shift, usually at the worst moment possible. This is why one failed check means reject, always.
How long is a shift? About 5-8 minutes. They ramp up - better disguises, more events.
How do I actually lose? Sanity hits zero, or three patients die in one shift. That's it.
Is it actually scary? The events will get you a few times. The rest is more tense than scary - think Papers, Please with jumpscares.
